POSITION DESCRIPTION:
Under the general supervision of the Program Director, the Registered Nurse (RN) provides high quality nursing care to Elderday Adult Day Health Center program participants.
The RN provides health care assessments, support services, and appropriate emergency interventions, oversees clinical treatment provided by the Licensed Vocational Nurse, as well as personal care services provided by program aides.
The RN also performs other duties as assigned.
DUTIES &
Responsibilities:
Participant Case Assessment and Management:
Conducts initial, six-monthly, and quarterly nursing assessments of program participants and ensures that the nursing component of the Individual Plan of Care is completed in a timely and accurate manner; completes required quarterly progress notes; monitors participants' health; develops and implements nursing care plans; liaises with participants' caregivers and primary care physician; provides ongoing nursing case management.
Monitors participant's health status on an ongoing basis including, but not limited to changes in physical, emotional and/or behavioral status; notifies primary physician of marked changes in participant's conditions.
Collaborates with program Social Worker to coordinate referrals to Adult Protective Services and other community services as appropriate.
Monitors health related absences of Elderday participants, working with the individuals, their families, and heath care providers to expedite their return to the program.
Refers participants to appropriate community health care services; acts as liaison with such providers; is responsible for physician and family contact and follow-up.
Provides health education to participants and their caregivers.
Works closely with Registered Dietician and Activity Coordinator to ensure that nutritional needs of participants are met and dietary restrictions followed; refers to registered dietician when appropriate.
Maintains high quality participant records, using good English grammar, in accordance with state regulations, including case management records, progress notes, care plans, accident and abuse reports.
Participant Medication Administration:
Monitors administration of and/or administers participant medications.
Monitors medication status of participants, including observation of potential over-medication or misuse of medication.
Coordinates with primary care physician for medication changes, as appropriate.
Professional Conduct:
Participates actively and arrives prepared for Interdisciplinary Team Meetings.
Remains clinically current on issues relating to geriatric health, laws and nursing practices.
Communicates effectively and respectfully with people, including participants and staff, from different racial, ethnic and cultural groups and from different backgrounds and lifestyles; demonstrates a knowledge and sensitivity to their needs.
Maintains confidentiality and treats Elderday participants and staff with dignity and respect at all times.
Performs other duties as assigned.

OTHER JOB DETAILS:
This is a FLSA non-exempt position.
The position is eligible a $.
40 per hour bilingual differential after passing a test administered by the HR department.
This position requires regular on-site attendance.
MINIMUM
Qualifications:
Knowledge of:
__ Nursing principles and practices, with particular reference to the aging; Medications of the aged, including uses, abuses, effects and adverse reactions; Physical assessment techniques; Case planning and management methods; Community health services and facilities that are available to seniors and their families; Public health nursing issues; and Medical terminology and equipment.
Ability to:
Understand geriatric nursing practices; Assess and monitor the health of elderly persons; Devise and follow through on health care plans; Understand accepted methods of practice within the RN's ethical and legal framework; Provide education to maintain the optimum health of participants; Work cooperatively with other staff as a team member; and Communicate tactfully, cooperatively, and effectively with program participants and their families, physicians, other agency staff members, representatives of other human services and community organizations, and the public.
OTHER REQUIREMENTS:
Must possess a valid California RN license.
Must possess or be willing to obtain a CPR certificate within 30 days of hire.
Must have a valid CA Driver's License, have access to drive a motor vehicle incidental to the performance of the work, and be insured.
Three years' experience in clinical practice with geriatric population strongly preferred.
Must pass a criminal background check and maintain a clean record.
Must test for TB and clear before first date of employment.
Must receive an annual influenza vaccination or be willing to wear a protective face mask during government regulated influenza season.
Bilingual English/Spanish preferred.
PHYSICAL DEMANDS:
Standing walking, sitting, typing, reaching, considerable strenuous physical exertion such as repeated lifting and/or carrying of items weighing 50 pounds or greater.
Must have the stamina to stand for long periods of time and sufficient strength to lift.
